What type of rods did you use?
How long was her hair?

If I was to do a spiral perm, I would not choose an acid perm. They are more gentle, yet they they also do not produce as strong a curl. My guess is that the curl formation was quite weak before she left the salon. Do you have any pictures?
You should have her come back in and shampoo her hair and look at the curl yourself. But you must be honest with yourself. Do you really think the curl was strong before she left the salon?
Much of the time that perms are not a success is the cause of the stylist. They do not prepare the hair nor do they rinse or blot properly.
You are the only one who knows if all these things have been done properly.
Tension is also a key factor. Was the hair wrapped neatly and evenly on the rod?
Are you sure that all of the hair was saturated? I always squeeze solution in that little hole on the end of the rod.
